Admissions

Applications for admission are now being accepted. We have openings for both male and female residents. If you are interested in further information, you may contact us by phone, e-mail or in writing. You may also fill out an application by downloading it from the box on the right.

Eligibility

The basic eligibility requirements* for Jacob's Village are:

• 21 years of age
• Primary diagnosis of mild to moderate
  developmental disability, cognitive challenge or related condition.
• Able to ambulate and transfer independently
• Independent with activities of daily living
• Minimal medical needs

* Note: These are general guidelines for admission eligibility; all prospective residents will be reviewed by our Admissions Committee. Jacob's Village hopes to serve adults with additional special needs as it develops.

Upon submission of the preliminary application form and review of materials by the Admissions Committee, prospective residents and their parents or guardians are contacted for a personal interview and a site visit.

Prospective candidates who remain interested in residency and are deemed potentially eligible are requested, along with their parents or guardians, to fill out a full application, provide supplemental materials, and complete a pre-admission weekend stay and evaluation.

Fees cover a portion of the expenses incurred for residency at Jacob’s Village, with additional support for Jacob’s Village provided by individual, corporation, and business donors. Residency costs are set by Board action and are evaluated annually.
